Following are forms for new counseling clients. They are provided for your convenience so that you may complete them prior to your initial appointment. You will need a free PDF reader like Adobe Acrobat Reader to view them. Most computers/web browsers come with this already installed, but a link is provided for you in case you need to install it.
IMPORTANT Several of these forms are fillable PDFs so that you may complete them on your computer and then print. Please note that you can save the original form to your hard drive, however you cannot save the information that you fill out without additional software. For this reason, please allow yourself enough time to complete the form and print it in one sitting. You may also prefer to print the form out and complete it by hand.
